Emergency Authorization Form CPR & DNR CONSENT FORM
Longevity Veterinary Center provides urgent and emergency care during all regular hours and has now expanded its operating hours to better serve the needs of pet owners in the community. Whether it's a sudden illness, injury, or any other urgent care need, pet owners can now rely on the expertise and dedication of the center's veterinary team until midnight, seven days a week, for pets of all kinds including exotics.
Common urgent conditions that may require immediate veterinary attention include:
• Traumatic injuries, such as fractures, wounds, or animal attacks
• Difficulty breathing or severe respiratory distress
• Uncontrolled bleeding or hemorrhage
• Seizures or loss of consciousness
• Ingestion of toxic substances or foreign objects
• Sudden onset of severe pain or discomfort
• Heatstroke or hyperthermia
• Allergic reactions, including swelling, hives, or anaphylaxis
• Acute abdominal pain or bloating
• Inability to urinate or defecate
• Sudden weakness or lethargy
When it comes to your animal's urgent health care needs, calling ahead isn't always possible so our doors will be open nightly until midnight. If you are able, please call ahead to help us prepare for your animal's arrival.
